Hourly Pay Range (Minimum - Midpoint - Maximum):

$23.000 - $23.000 - $26.777


The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.



What You Will Do
Collaborate with the interdisciplinary teams in the delivery of quality patient care under the direction of a registered nurse, assists in performing personal care needs and compassionate end-of-life care. Contribute to overall unit functions.

Required Qualifications
  • California Home Health Aide (HHA) - CA Department of Public Health -REQUIRED
  • California Certified Nurse Assistant (CNA) - CA Department of Public Health -REQUIRED
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED
  • Driver's License - CA Department of Motor Vehicles -REQUIRED
  • ServSafe Food Handler Certified - National Restaurant Association -REQUIRED

Preferred Qualifications
  • 1 Year experience in acute care, home health, or hospice.

Other Qualification Requirements
  • Current Food Handler card or acquisition within 90 days of hire. Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines.

Essential Functions
  • Plan of care
    Observes patient condition and reports changes to the RN.
    Reports pertinent patient related information to RN.
    Observes changes in patient's condition such as declining ability to participate in ADL's, changes in skin condition, level of consciousness, pain management, etc.
    Promptly reports appropriate changes in patient's status to RN.
    Effectively communicates and participates in the coordination of the plan of care to meet established goals.
  • Departmental compliance
    Performs routine equipment maintenance according to departmental policy.
    Maintains all certifications required of the job.
    Demonstrates and maintains expertise in through attendance and participation in department in-services and meetings.
    Current CPR Certificate

    Fire
    Hazardous Materials
    Infection Control
    Attendance at unit specific mandatory in-services.
    Mandatory/non- mandatory staff meetings: reviews staff meeting minutes.
    Attendance at additional in-services per unit standard and expectation.
    Monitors and maintains CEU record per department policy and expectation.
  • Effective communication
    Understands and demonstrates effective communication to patients/families in alignment with hospice philosophy and patients plan of care.
    Provides appropriate discipline specific information on end of life issues to patient/families based on their needs.
    Listens actively and provides a meaningful presence to facilitate the identification and discussion of patient goals and preferences as it relates to personal care.
    Promotes rapport to foster patient relationship to support patient throughout their hospice care.
  • Patient care
    Provides comprehensive personal care to patients under the supervision of the RN. Appropriately interacts with patients/families.
    Adheres to plan of care.
    Demonstrates functional knowledge and skills at a competency level.
    Performs comfort and safety measures to ensure that patient's needs are met.
    Performs simple nursing procedures under the supervision of the RN (i.e. vital signs): 1) Basic patient care; 2) Toilet, eliminating; 3) Transfer techniques; 4) Admission duties.
    Promotes and develops relationships based on trust, respect, and rapport.
    Maintains privacy and confidentiality at all times.
  • Professional development
    Demonstrates and maintains expertise through attendance and participation in department in-services and meetings.
    Maintaining individual in-service/performance records.
    Demonstrating process of self-discovery including knowledge of strengths and weaknesses and view deficiencies as opportunities for growth.
  • Safety
    Maintains a safe working environment for patients, families and other members of the team.
    Complies with universal precautions and Department infection control policies.
    Consistently utilizes proper body mechanics and available support equipment to ensure patient's safety and avoid personal injury.
    Identifies and communicates patients who are at risk for injury to self and others.
  • Teamwork
    Understands and demonstrates the team member role and responsibilities.
    Demonstrates effective and positive working relationships.
    Conveys a willingness to help and promotes cooperative team efforts.
    Demonstrates an ability to perform and effectively communicate with other team members.
    Contributes ideas/suggestions to improve quality comfort care needs of the patient.
    Elicits assistance from team members to problem-solve and respects the opinions from others.
    Participates in shift reports, effectively communicating discipline specific information.
    Appropriately refers to appropriate discipline for matters outside their scope of practice.
    Provides assistance with patient care and environmental duties as assigned.

  • Knowledge of medical terminology and techniques.
  • Ability to execute Home Health Aide skills safely.
  • Ability to effectively communicate and function in a team environment to enhance patient care.
